Job Summary and Qualifications

What you will do in this role:

You will be responsible for coordinating the nursing care of assigned patients in the Unit in order to provide quality patient care.
You will be responsible for knowledge and skill necessary to provide care based on physical, psychological, educational, safety and related criteria for the appropriate age populations.
You will provide professional nursing care exercising independent judgment in utilizing the nursing process to assess, plan, implement, and evaluate.
You will provide leadership by working cooperatively with ancillary nursing and other patient care team personnel in maintaining standards for professional nursing practice in the clinical setting.

What qualifications you will need:

Graduated from an accredited school of nursing with an Associates or Bachelor of Science in Nursing (BSN) is preferred.
Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date).
American Heart Association BLS/ACLS Certifications required

Job Summary and Qualifications As an Oncology Nurse Navigator, you will function as a member of the multidisciplinary team and serve as an advocate and educator for cancer patients from diagnosis through the end of active treatment. You will ensure the patient remains compliant with the treatment plan. You will achieve this goal by building relationships with patients and physicians and coordinating the plan of care, education, resource provision , and/or representation within the multidisciplinary care environment. You will also assume responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for cancer patients, including support with interdisciplinary and cross-facility coverage and collaboration . What you will do in this role: You will serve as a liaison between the patient and family with the multidisciplinary healthcare providers to ensure effective communication and collaboration. You will collaborate with local physician liaisons and leadership to conduct internal and external outreach and marketing when appropriate You will serve as a patient advocate from diagnosis to end of active treatment You will assess patients’ medical, social, and psychosocial needs/barriers to care and link them to appropriate resources. You will provide appropriate teaching , outreach, and support to patients and families and ensure the patient is empowered to manage their own healthcare needs . You will conduct follow-up conversations with all patients and communicate any concerns, changes, or social needs to the appropriate physician or other care providers . What you will need in this role: Three to five years oncology nursing experience Experience in program development preferred Experience as a nurse navigator and/or patient educator in oncology preferred Experience in leadership and customer service – preferred Bilingual in Spanish preferred Education Associate’s Degree in Nursing required Bachelor’s Degree in Nursing preferred Licenses and Certifications Basic Cardiac Life Support (BCLS) certification Current licensure as a Registered Nurse (RN) Oncology Certified Nurse (OCN) achieved or in progress – preferred Sarah Cannon is the Cancer Institute of HCA Healthcare.
