Description

This RN position supports the Transfer Referral Center at UH Management Services Center in Shaker Heights.  The position is full-time, 36 hours per week, on evening shift.

Responsible for performing aspects of the Patient Flow Management process ensuring that transfers, patient placement and related activities occur in a timely, safe and accurate fashion. This includes providing clinical assessment for patients being placed through the Center for Patient Flow Management. 

Position Summary/Essential Duties:  

  1. Coordination of incoming patient transfer requests with receiving facility, physicians, and necessary transport resources  

  1. Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes  

  1. Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations  

  1. Displays leadership that influences optimal clinical and operational outcomes  

  1. Delivering exceptional customer service while facilitating communication across multiple disciplines 

  1. Possess and obtain working knowledge of external and inter-facility protocols and standards of practice  

  1. Work with all Center for Patient Flow Management (CPFM) staff in a team orientated, collaborative approach 

  1. Responsible for promoting a Best Place to Work Culture and positive and professional work environment 

  1. Radio communication and coordination with multiple agencies, including Air Medical programs and UH Critical Care Transport 

  1. Dispatch of UH Critical Care Transport ambulances  

  1. Other duties as assigned 

A Brief Overview

Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
