FAMILY ORIENTED ASSISTED LIVING LOOKING FOR AN LPN
Summary: The LPN Wellness Coordinator is responsible for the coordination of personal care and medication administration programs within an assisted living environment. Responsibilities include but not limited to the hiring, training and supervising of HHA/PCAs, Medication Care technicians while being compoliant with OMIG and NYS DOH regulations. The Wellness Coordinator needs to be familiar with state regulations and Promenades Policies and Procedures.
Duties / Responsibilities
- Accurate transcribing orders, working with resident doctors and pharmacies to ensure the MAR's are up to date at all times
- Executing personal plans of care for the resident population within the community
- Assisting with personal care needs of residents as needed
- Documentation of case management notes with respect to resident care activities and incidents as needed.
- Dispense medication to residents as per the Medication Administration Record
- Recruit, hire supervise and coach personal in the medication program.
- Ensure that the Medication Administration Policy and Procedures are followed.
- Ensure that all changes to the MARS has corresponding documentation in the resident records.
- Regularly monitors each resident’s medication, MARS and treatment profile to ensure that each medication/treatment is administered as ordered, is available and is documented correctly.
- Assists with medication administration as well as plan of care tasks, as needed.
- Orders new medication and refills as needed.
- Coordinate medical appointments as needed and helps in attaining annual medical evaluations.
- Coordinate care with residents, family members and physicians.
- Ensure adequate staffing scheduling and completion of daily medication administration.
- Ensure all resident and staff regulatory requirements are met related to Medicaid, OMIG, and NYS DOH.
- Take on manager on duty and on-call responsibilities as needed.
- Other tasks as assigned.
Qualifications:
- Maintain a current NYS license as a Licensed Practical Nurse
- Supervisory and management experience
- Meet case management requirements of assisted living NYS regulations
- Excellent verbal and written communication skills
- Has good organizational and time management skills
- One (1) year experience with seniors in a long term care or assisted living setting is preferred
- Ability to effectively read, write, and communicate in English
- May require lifting or bending. Must be able to lift 25-35 pounds
