Responsibilities:

Provide direct patient care to residents, including administering medications, managing wounds, and monitoring vital signs
Develop and implement individualized care plans to meet the unique needs of each resident
Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services
Conduct patient assessments and evaluations to identify potential risks and opportunities for improvement
Communicate effectively with residents, family members, and other healthcare providers to ensure seamless transitions and optimal care outcomes
Participate in quality improvement initiatives to promote evidence-based practice and patient safety

Requirements:

Current licensure as a Registered Nurse in the state of Ohio.
1 year of experience as a Registered Nurse preferred but not required (entry-level opportunity available).
Ability to lift, transfer, and reposition patients as needed.
Strong communication and interpersonal skills.
Ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
