Care Coordinator / LPN ( Licensed Practical Nurse ) in Assisted Living

River Falls Senior Living is searching for a motivated Licensed Practical Nurse (LPN) to help carry out our core values of making life fun, making the community a home, and delivering purpose to everyone.

As an LPN, you will work with the Interdisciplinary team, resident, and family to assess and determine needs within an Individual Service Plan (ISP) and coordinate service provision as identified in the ISP. The LPN is responsible for working with the Wellness Director, Resident Care Assistants, and Resident Care Supervisors to provide optimum care for each individual resident. 

LPN Essential Duties and Responsibilities:

• Team First. Accepts assigned duties in a cooperative manner. Performs other related duties as directed by Wellness Director.
• Role Model: Assisting staff in the provision of direct care to residents affords not only the opportunity to serve as a role model but also provides the LPN the opportunity to assess Resident Care Assistants skills and a better recognition of the supports and constraints under which the staff function. 
• Resident Advocate: The LPN functions as liaison between staff and administration, staff and families, staff and service providers including physicians to promote optimal care for the resident on both a formal and informal basis.
  • This includes:
    • Evaluating potential residents
    • Implementing the Individual Service Plan for each resident
    • Coordinating and working with Home Care Services such as VNA , PT & OT
    • Identifying safety hazards and initiate corrective action as necessary.
