Responsibilities*

  • Provide the delivery of culturally sensitive, age-specific team-based care models in delivering nursing care for Primary Care patients including adult medicine, pediatric, Palliative Care, and OB/GYN patient populations.
  • Perform phone triage, including assessment, screening, clinical advice, nurse visits (in person/ video visit) in-clinic hands on procedures, and other duties assigned.
  • Accurate and efficient documentation of nursing care.
  • Knowledge and appropriate use of internal and external resources.
  • Provide functional supervision to assistive personnel; collaborate with colleagues, other disciplines, and health care/community services to promote continuity of care.

Required Qualifications*

  • Three (3) yrs (out of last 7 years) of RN experience in Adult Internal Medicine (to include Adult Inpatient, Emergency Services, or Ambulatory Care Medicine Adult).
  • One (1) year (out of last 7 years) of RN experience in Pediatric Medicine (to include Peds Inpatient, or Emergency Services, or Ambulatory Care Medicine Peds).
