LPN MDS Full-time

We’re Hiring!
Are you an experienced RN or LPN with expertise in the Minimum Data Set (MDS) process? Join our team as an MDS Coordinator in a dynamic, collaborative environment where you’ll play a key role in ensuring timely and accurate completion of the RAI process and care management from admission to discharge.

What You’ll Do:

  • Collaborate with the Interdisciplinary Team to assess resident needs and create MDS assessments and care plans in accordance with state and federal guidelines.
  • Ensure accurate and timely documentation of the MDS process, including coding, medical record reviews, and interdisciplinary updates.
  • Assist in coordinating daily stand-up meetings and review care and assessment schedules.
  • Stay updated on federal, state, and certification guidelines to ensure compliance with all regulations.
  • Submit resident data to state and federal agencies, and ensure all MDS documentation is accurate and complete.

What We’re Looking For:

  • Graduate of an approved Registered Nurse (RN) or Licensed Practical Nursing (LPN) program and licensed in the state of practice.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
  • In-depth knowledge of the MDS 3.0, Medicare PPS process, Medicaid reimbursement, and OBRA regulations.
  • Experience with care planning and quality indicators.
  • Strong attention to detail and excellent communication skills.
