As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Milbank  by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

  • As a minimum, an unencumbered State of South Dakota R.N. or L.P.N License;

  • Be a graduate of an accredited nursing program and C.P.R. Certification;

  • Prior experience as a MDS Coordinator and at least one (1) year of experience as an R.N. or LPN in a Skilled Nursing Facility setting is preferred.

Position Summary The MDS Coordinator is responsible for managing and completing resident assessments in accordance with federal and state regulations.
