Summary

Provides chronic condition management and health coaching to members with disease risk indicators to achieve health risk reduction, improved quality outcomes, enhanced member experience, and cost-efficiency.

Formal Education Required:
Bachelor’s of Science in Nursing (BSN) or equivalent education and experience.
Registered Nurse (RN) state license in good standing.
Experience & Training Required:
A minimum of three years experience to include care coordination, chronic condition management, acute or managed care.
