Responsibilities

As a Home Health Registered Nurse, You will:   

  • Assess/monitor physical, emotional, and psychological needs of patients
  • Create home health care plans that align with MD orders and the patient's goals
  • Direct nursing care: administering medications, treatments, and interventions
  • Provide pain and symptom management
  • Educate and support the patient’s family and caregivers
  • Collaborate with an interdisciplinary team
  • Maintain accurate and timely documentation
  • Participate in on-call rotation as required by the local branch

Qualifications

  • Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the applicable state is required 
  • One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred 
  • Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy 
  • New nursing graduates may be considered in select markets based on program availability 
  • Current CPR certification

KEY RESPONSIBILITIES: • Provides competent nursing care & interventions to home health patients according to state and industry and profession • Supervises care provided by Home Health Aides, Licensed Practical Nurses (LPN) assigned to the care team (completing supervisory visits of HHAs as required) • Helps maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan of care, making referrals to other disciplines as needed • Demonstrates intermediate knowledge of home care and competency in discipline for specific patient care skills, required for the provision of care • Conducts field visits to (1) evaluate & initiate start of care and (2) conduct clinical outcomes monitoring, follow-up and care performance improvement initiatives • Utilizes accepted company teaching materials in patient/family education • Assesses, monitors, documents and reports progress of patient's health and condition using required documents via clinician preferred technology, and within established timelines • Completes documentation timely, accurately, and at the point of care, according to industry standards (e.g., OASIS assessments, SOC/Admissions, Recertifications, ABN’s, subsequent visits, physician orders, care coordination, etc.) • Honors patients/residents’ rights to be notified in advance of care, treatment and services to be provided and works with patient team to adhere to physician-approved care plan. Ensures equitable treatment, self-determination, individuality, privacy, property and civil rights. • Complies with corporate compliance program • Meets agency productivity standards and utilizes time and resources effectively and efficiently • Participates in center/agency surveys (Licensure/Joint Commission), mandatory in-services, case conferences, staff meetings, and programs (e.g., Performance Improvement Program) as required • Follow established safety regulations and procedures, and report job-related functions/tasks that involve occupational hazards, as necessary • Maintains current license, certification, and clinical records to meet federal, state and agency regulations and guidelines • Ability to be self-directed, prioritize, manage time appropriately, and meet outcomes and strategic goals by targeted deadlines. Registered Nurse - Night Shift | Carolina Dunes Behavioral Health | Leland, North Carolina About the Job: The Registered Nurse is responsible for providing nursing care to patients. Supervising care treatment and services of patient care provided by LPNs, CNAs and other staff on assigned unit and shift. Developing, implementing, and evaluating an individual plan of care for each patient. Assessment and re-assessment of patients. Communicating appropriate information with the interdisciplinary team. Reporting patient concerns and changes in condition to supervisor and/or physician in a timely manner. Roles and Responsibilities: ESSENTIAL FUNCTIONS: · Provides and coordinates care by assessing physical and behavioral health needs of patient, develops and implements nursing care plans, maintains medical records and educates patients and their families about various physical and behavioral health conditions while ensuring patient safety at all times. · Assesses patient’s status on an ongoing basis and pertinent information gathered is documented. · Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission. · Takes physician’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews physician’s orders. Ensures physician’s orders are legible if taken verbally and transcribed appropriately per policy and procedure. · Accurately administers medications per ordered note and documents accurate count of controlled substance. Adjusts medications under provider’s supervision as needed; monitors lab results to validate medication compliance. · Notifies physician of significant changes in status such as difficulties with detox, medical emergency or change in medical status regarding response to medications, etc. · Supervises self-administration of medications using safety initiative of using two means of patient identification: documents and reports effects of scheduled and PRN medications. · Maintains accountability of administration, recording, control and security of medications, including narcotics. · Completes admission and discharge processes efficiently and accurately as directed. · Confers regularly with treatment team (e.g., nurses, therapists, program managers, physician) regarding patient care. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc. · Provides education/explanation to patients, families, and co-workers regarding medication usage/maintenance, common side effects, patient's rights, as well as other topics necessary for optimal patient care. · Monitors patient acuity, census, staffing needs, and makes arrangements for coverage as needed, following established guidelines. · Assures thorough, accurate documentation is completed for all patients on each shift and patient assignment sheets are completed daily and tasks are carried out as assigned. · Gives thorough report to oncoming staff, supervisors, and physician. · Provides coverage in emergency services as needed. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: · Graduation from an approved/accredited school of nursing. · One or more years’ nursing experience within clinical area of service provided by facility preferred. · Experience in behavioral health/psychiatric/addiction recovery environment preferred. LICENSES/DESIGNATIONS/CERTIFICATIONS: · Valid current RN license from the state or from a compact state, where applicable. · Certified in Psychiatric Nursing preferred. · CPR and de-escalation and restraint certification required (training available upon hire and offered by facility). · First aid may be required based on state or facility requirements.
