Summary:
An Advanced Practice Provider functions as part of the interdisciplinary healthcare team in accordance with privileges approved by the credentialing committee to provide high quality, cost effective care to patients within the APPs scope of practice in collaboration with a supervising physician. The APP reflects the mission, vision, and values of the organization, and complies with all relevant policies, procedures, guidelines and other regulatory and accreditation standards.


The Advanced Practice Practitioner of UNC Health’s Advanced Care at Home program is a dynamic, experienced provider who is passionate about transforming the future of healthcare. This role provides the following opportunities:

·Work in a team environment to provide high-quality inpatient care to patients in their own homes through Advanced Care at Home (ACH).

·Clinical outcomes are equal to or better than inpatient hospitalization with improved patient experience

·State-of-the-art equipment at Command Center and patients’ home

·Patients have access to hospital services and more including medication management, mobile imaging, home infusion, meal delivery, nutrition consults, phlebotomy, rehab services

·Integrated care with primary care providers, specialists & ancillary departments

Responsibilities:

·Perform the initial virtual history and physical for new patient admissions after patient arrives home

·Collaborate with the supervising physician to develop and implement a plan of care, including diagnostic and specialty consultations virtually or as day trips to brick and mortar facilities

·Travel to patient’s home and perform in-person assessments, as clinically indicated

·Provide safe handoffs to the attending physician and Command Center Registered Nurse, as well as patient’s primary care physician during hospitalization as needed and at discharge

·Perform clinical screenings on eligible patients at eligible brick and mortar facilities

·Communicate with the patient and family concerning the ACH model, incorporating patient and family values and goals in the plan of care

·Promote a collaborative relationship between the physicians, Command Center Registered Nurse, and supplier providers

·Maintain and practice in accord with practitioner guidelines per state and hospital regulations

    ·Command Center – Parkline - is considered “place of business” for this role located in Chapel Hill, NC. The candidate must be "local" however the role is in a hybrid capacity.

    Licensure/Certification Requirements:
