We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
Manage Residents with acute change in condition.
Advanced care planning and chronic care management.
Prescribe appropriate medications, including controlled substances in accordance with NH Law.
Order, review and/or follow-up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
and/or the care plan.
Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

Current, unrestricted NP license in the state of New Hampshire.
Board Certification.
DEA licensure.
Clinical background in adult, family or geriatric specialties.
Experience working in a long-term care setting.
Ability to work with an EMR.
Ability to manage patients with advanced illness and multiple chronic conditions.
Ability to work within an interdisciplinary care team to coordinate patient care.

We are seeking a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing Endocrinology practice. This role offers the opportunity to care for patients with a wide range of endocrine disorders , including diabetes , thyroid disease , adrenal and pituitary disorders , osteoporosis , and more. A strong emphasis is placed on patient education , particularly in diabetes self-management and lifestyle interventions. Key Responsibilities: Provide comprehensive evaluation, diagnosis, and treatment plans for patients with endocrine conditions. Manage patients with type 1 and type 2 diabetes, including insulin management and diabetes education. Conduct follow-up visits and monitor lab/imaging results to adjust care plans as needed. Collaborate with endocrinologists, dietitians, and other specialists to ensure high-quality, coordinated care. Educate patients and families on disease processes, medication use, and lifestyle changes. Participate in ongoing quality improvement and clinical development initiatives.
