General Summary/Responsibilities

Premier Community Health, a division within Fidelity Health Care (FHC), provides comprehensive health

and wellness services to employer worksites and the community. This position requires travel to various

sites in Southwest Ohio for health and wellness events. Community Health Screeners work at events to

provide health screenings, results counseling, and/or vaccinations (flu and VFC) to employees and

community members, assisting Premier Community Health to accomplish its mission to build healthier

communities. Community Health Screeners perform a fingerstick test to determine health measurements

for participants. The candidate must aspire to become proficient and maintain clinical competency within

PCH standards and possess excellent interpersonal and customer service skills, and a teamwork mentality.

Visit Nurse is responsible for the provision and coordination of all components of home care services for patients. The HCCM visit nurse will provide care as ordered by the physician, follow policies, procedures, conditions of participation, and regularly report to their supervisor. The incumbent will facilitate the development and implementation of the plan of care as outlined by the physician. In addition, the incumbent will be responsible for all required documentation related to the care delivered and will communicate pertinent information regarding the patient to all appropriate health care team members.

Licensure/Certification/Registration

 Current Ohio Registered LPN license in good standing.

 CPR Certification

Experience

Minimum Level of Experience Required: 1 - 3 years of job-related experience

Preferred experience: One year clinical experience in acute care setting preferred

Knowledge/Skills

 One-year clinical experience in acute care setting preferred

 Clinical experience must have occurred within the last three years unless employed currently in home care setting

 If working as a specialist, documentation of the skills and certifications must be provided

 Must be capable of standing, stooping, bending and lifting up to 40 lbs.

 Must be self-directed and able to work with little supervision

 Computer Skills Preferred

 Home care experience preferred

 Must be proficient at fingerstick test/procedure.

▪ Must be proficient in conducting blood pressure screenings.

 Must be able to use equipment to obtain health screening results.

 Must demonstrate the ability to maintain confidentiality of participant information/data.

 May be required to offer a quick counsel on health screening results.

 May be required to administer vaccinations (flu and COVID).

 Must have strong people skills, reliability, professionalism, and the ability to work under pressure.

 Must be proficient with basic computer skills – Analytics software and Epic may be used.

 Must have reliable transportation, accurate contact information and a valid Ohio driver’s license.

 Must be able to lift at least 10 pounds when transporting supplies, and able to walk moderate

distances to screening spaces.

 Must have regular access to email and internet to check communications
