The registered nurse plans, organizes and directs hospice care services specific to each patient/family which includes instruction and evaluation in the home. The professional nurse is accountable to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. PRIMARY FUNCTION

The Admissions Registered Nurse is an Interdisciplinary Team member whose primary responsibility is to present ANCHOR HEALTH services and philosophy to patients and families in a manner that results in the admission of appropriate patients.

JOB RESPONSIBILITIES FOR THE ADMISSIONS REGISTERED NURSE

1. Admissions Registered Nurse participates in the information gathering, analysis, education, and form completion aspects of the admissions process

2. Admissions Registered Nurse evaluates patients for hospice appropriateness, e.g. medical records review as requested.

3. Admissions Registered Nurse Obtains agreement for admission of referred patients and families who meet admission criteria.

4. Admissions Registered Nurse analyzes data collected by the admission staff or ANCHOR HEALTH clinicians to facilitate/obtain admission orders

5. Admissions Registered Nurse builds effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HEALTH.

6. Admissions Registered Nurse maintains clinical and professional competence.

7. Admissions Registered Nurse takes the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.

8. Admissions Registered Nurse utilizes interpersonal skills to help appropriately educate and address any questions the patient and/or patients’ family has concerning hospice and palliative care.

9. Other duties and responsibilities as assigned

1. Admissions Registered Nurse has knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas.

3. Admissions Registered Nurse has the knowledge of clinical manifestations of diseases and ability to develop ANCHOR HEALTH specific clinical knowledge.

5. Admissions Registered Nurse has reliable means of transportation.

The Admissions Registered Nurse reports to the Admissions Manager designated in the location. In the event the location does not have an Admissions Manager, the Admissions Registered Nurse will receive direction from the Patient Care Administrator or program designee

QUALIFICATIONS FOR THE ADMISSIONS REGISTERED NURSE

1. Graduate of a National League of Nursing accredited school of nursing.

2. Current licensure in State of CA and CPR certification.

3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license.

Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.

4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Admissions Registered Nurse is expected to have strong presentation skills.

5. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching.

6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
