Provides for patient needs as recognized through use of the nursing process (assessment, planning, implementation, evaluation) and the mission statement; organization and supervision of care provided by supportive personnel; coordination of care provided by ancillary services. Including patients and significant others (SO)/families in developing an individualized plan of care. Education of patients/SO/families to their disease process and plan of care. Serves as patient advocate and responsible for providing care to patients requiring treatment of any medical, surgical, or other condition, including chronic medical and minor injuries or illnesses. Certification- ACLS required within 6 months for the Adult EDs and Free Standing EDs. PALS required within 1 year of hire for the Free Standing EDs and required within 6 months for WCH ED. Neonatal Resuscitation Program (NPR) is required within 6 months of hire at NAS ED.

Oversee the delivery of quality care, supervising clinical personnel and ensuring compliance with agency standards and healthcare regulations. Lead and inspire clinical teams and clinician retention. Operate in a dynamic environment, balancing administrative responsibilities with field-based interactions. Educate and support clinicians to promote effective performance and the delivery of high-quality home care services. Assist in the screening, interviewing, and hiring process for new personnel. Participate in the orientation and training of newly hired staff to ensure they are well-prepared for their roles. Ensure adherence to accepted professional standards and principles. Consistently follow and enforce agency policies and procedures to set an example for employees. Lead the team in alignment with the agency’s mission, vision, and values. Promote a culture of accountability and continuous improvement. Conduct ongoing assessments of clinicians to evaluate their understanding and compliance with policies and procedures. Utilize ride-alongs, elevate calls, and daily touchpoints as tools for evaluation and feedback. Participate in public relations and community activities to enhance the agency’s visibility and reputation as a valuable member of the healthcare delivery system. Conduct regular 1:1 meetings and ride-alongs with clinicians. Provide quarterly SHP scorecard reviews with clinicians and continuous education to ensure clinical excellence. Perform weekly field visits with clinicians to mentor, guide, and support their professional growth and clinical expertise. Instruct on the use of patient and physician portals, use of EMR system, and clinician scheduling tools to enhance communication and care coordination. Undertake other duties and responsibilities as delegated by the Director of Clinical Services. Qualifications Must have a current RN license (state-specific). Minimum two years of experience in home health/ Must have Oasis documentation experience Strong leadership and management skills. Excellent communication, organizational, and time management skills.
