PRN, RN or LPN Behavioral Crisis Center

Job Description:

The Licensed Practical Nurse (LPN) is responsible for providing nursing services to patients in residential care facilities, behavioral health and/or substance use disorder treatment programs. They work to maintain a safe and therapeutic environment for clients and provide support through various nursing duties across outpatient psychiatry, addiction, and housing services. Additionally, they work to build and maintain a strong working relationship between clients and clinic staff.

Essential Job Functions:

  • Administer medications to clients via various methods, including oral, injection, self-administration and others, according to service line requirements.

  • Perform nursing assessments, including physical and mental health assessments, and psychiatric screeners, according to service line requirements.

  • Document medication administration and nursing assessments accurately and completely, adhering to service line standards.

  • Conduct MAR reviews and medication counts, monitoring house stock of medications and refill status for housing clients. Prepare pass medications as needed.

  • Communicate with clients regarding medication questions and needs, staging refill requests to providers according to standard operating policies and procedures.

  • Room clients, verify client data and medical history, confirm purpose of visit, and obtain necessary physical preliminary requirements such as blood draw, urine sample, height, weight, blood pressure, pulse, and temperature.

  • Complete all required documentation in a timely manner, including nursing assessments, physical and spoken interventions with clients, nursing-led educational groups, and client safety plans.

  • Ensure that all medical and medication records are up-to-date and accurate, and assist with general lab and phlebotomy duties as needed.

  • Function as a resource person for nursing and medical issues, overseeing infectious disease control, client hygiene, and related client and facility health issues, under the supervision of nursing leadership.

  • Participate in all required meetings as assigned, and perform other related duties as required, assigned, and/or delegated by nursing leadership.

Knowledge, Skills, and Abilities:

  • Proficient in accurately entering, transcribing, recording, and storing information in various formats, including written and electronic forms.

  • Able to assess and provide feedback on programs and services to improve their effectiveness and efficiency.

  • Strong communication skills, both verbal and written, with the ability to produce clear and concise reports and convey complex ideas in a simple and respectful manner.

  • Committed to maintaining confidentiality and protecting sensitive information, including medical records, by using secure methods such as encrypted files and password-protected documents.

  • Adept at reading and interpreting various types of documents, including laboratory results, notes, and electronic health records, as well as safety rules, operating instructions, and procedural manuals.

  • Skilled at writing routine reports and correspondence that are clear, effective, and respectful, using multiple modes of communication, including in-person, phone, and video conferencing.

  • Competent in basic mathematical calculations, including addition, subtraction, multiplication, and division of whole numbers, fractions, and decimals.

  • Possesses strong problem-solving abilities, including the capacity to understand and follow instructions, and adapt to new situations and challenges.

  • Demonstrates a willingness to learn, grow, and work collaboratively with others, embracing diverse perspectives and approaches.

Experience and Education Qualifications:

  • Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.

  • Graduation from an accredited practical nurse program.

  • Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.

  • Completion of New Hire Orientation at the beginning of employment.

  • All training requirements including Relias at the beginning of employment and annually thereafter.

Physical Requirements:

Perform nursing assessments per service line requirements, utilizing a high level of clinical competence. Administer medications via various methods according to service line requirements, ensuring complete documentation. Communicate with clients regarding medication questions & needs, staging refill requests to providers. Room clients, verify data/medical history, and obtain necessary preliminary requirements. Complete required documentation in a timely manner, including nursing assessments and client interactions. Ensure medical & medication records are accurate and up-to-date, completing prior authorization paperwork as needed. Assist with general lab & phlebotomy duties while following infection control precautions. Interview clients and families to gather pertinent nursing & medical history. Participate in all required meetings and perform other related duties as assigned. Provide direct patient care, including assessing patient needs, developing care plans, implementing interventions, and evaluating responses. Monitor and record vital signs to assess patients’ health statuses. Maintain up-to-date, accurate documentation on patients’ charts and communicate effectively with other healthcare providers regarding changes in condition, treatment plan modifications, and progress reports. Perform routine wound care, cleaning wounds, and reporting any signs of infection or complications. Collect specimens for laboratory testing, following standard precautions to prevent contamination. Collaborate with other healthcare providers to ensure comprehensive and coordinated patient care. Educate patients and families about disease processes, self-care strategies, and community resources. Ensure patient safety by adhering to protocols related to fall prevention and emergency response. Uphold ethical principles, legal standards, and regulatory requirements governing nursing practice. Education, Experience, and/or Credential Qualifications: Valid licensure as a registered nurse (RN) or current licensure as a Licensed Practical Nurse (LPN) in the state of practice; one year of experience preferred for RN. Completed graduation from an accredited practical nurse program for LPN. Ability to read & interpret documents such as lab results and procedure manuals. Strong communication skills to effectively interact with clients, families, and other healthcare providers. Ability to follow direction and accept supervision.
