Position Summary:

Functions as a total resident care giver, utilizing the nursing process in meeting the health care needs of residents in accordance with the Michigan Nurse Practice Act. Maintains resident privacy, dignity, confidentiality, and respects resident rights.

Essential Functions and Responsibilities:

  • Performs purposeful hourly rounding including Pain, Position, Personal needs and Possessions
  • Applies the nursing process in provision of resident care
  • Collaborates with interdisciplinary team in the formulation of a resident plan of care and its updating; and participates in team and family conferences as needed
  • Provides nursing care to residents wound care, administers medication, etc.
  • Implements the residents’ plan of care and enforces hospital policies
  • Completes documentation forms and inputs data as required
  • Demonstrates professionalism in the written and oral communication with residents, visitors, coworkers, physicians, and other health care members
  • Demonstrates knowledge of correct operation and maintenance of equipment utilized for delivery of resident care, striving to prevent potential hazards to residents, visitors, and staff members
  • Demonstrates organizational ability in obtaining and utilizing time, equipment, supplies, and resources necessary
  • Performs resident discharge planning
  • Encourages independence by residents in activities of daily living to the fullest extent possible
  • Performs other department related duties as assigned

Qualifications:

Required:

  • Graduate of approved school of practical nursing
  • Licensed by State of Michigan
  • BLS Certification

Preferred:

  • Previous experience in long-term care or acute care setting

The LPN demonstrates knowledge of the medications he/she administers, including actions and side effects, and instructs the participant/family in safe administration of medications at home. The LPN signs, dates, and performs all charting/documentation of medications; the LPN reports and follows up on any medication errors in accordance with the policies and procedures of PACE; the LPN participates in the collection of information and maintains accurate records of participant functional status and care given. The LPN advocates to others on behalf of the participant and demonstrates accountability in resolving participant concerns or issues. The LPN understands, complies with and promotes the Participant Bill of Rights and assesses and works toward achieving high levels of participant satisfaction. KNOWLEDGE, SKILLS, AND ABILITIES: The LPN participates in annual, mandatory in-service training and screening, including but not limited to: infection control, TB testing, safety training, and BLS training. The LPN assumes responsibility for self-development through continuing education, utilizing resources, within the health care system or elsewhere. Must be a Licensed Practical Nurse with current Michigan licensure. The ability to establish and maintain interpersonal and interdisciplinary relationships. The LPN assists the RN with the implementation of nursing research studies through gathering of data.
