Are you an experienced Registered Nurse (RN) with a passion for accurate documentation and high-quality resident care? We’re seeking a Traveling MDS Coordinator to join our dynamic clinical support team, providing critical MDS and care planning support across 20 skilled nursing facilities throughout Ohio.

What You'll Do
As a Traveling MDS Coordinator, you'll work closely with local facility teams to:

Complete and coordinate MDS assessments and care plans according to CMS and state regulations

Provide temporary MDS coverage during staff transitions, leaves, or audits

Support and train facility-based MDS staff as needed
-
Assist with quality measures, documentation accuracy, and compliance readiness

Travel regularly to facilities throughout Ohio (company-paid travel)
Key Qualifications
-
Active RN license in the state of Ohio (required)

Minimum 2 years of MDS experience in a skilled nursing or long-term care setting

In-depth knowledge of MDS 3.0, care planning, and RAI process
-
Strong understanding of Medicare/Medicaid reimbursement and case mix index
-
RAC-CT certification preferred (or willingness to obtain)

Ability to travel statewide up to 90% of the time (travel expenses reimbursed)

Excellent interpersonal, training, and time-management skills
- Additional duties as assigned by job description, regional support team, and CEO.
