Licensed Practical Nurse- Hospice (Field)
The Field Licensed Practical Nurse provides direct patient care to hospice patients in a variety of settings and levels of care under the direct supervision of an RN. Days/Nights/Weekends/Holidays as assigned to provide 24 hours a day, 7 days a week care assigned to a variety of teams/duties.
Responsibilities include:
• Providing care to the terminally ill patient
• Reinforcing patient and family training and education
• Performing patient assessment and documentation
• Participating in the comprehensive Interdisciplinary Plan of Care
• Participating in the Quality Improvement activities
All services will be provided 24 hours a day, 7 days a week as assigned to a variety of teams and duties. Nights and weekends. On-Call rotations as assigned.
JOB QUALIFICATIONS:
- Valid Driver's license with a good driving record; Personal car in good working order
- Valid Florida license for Licensed Practical Nurse (LPN)
- BLS and IV certification per department requirement
- Minimum 1 year experience in Medical Surgery Nursing or equivalent
- Hospice experience preferred
- Graduated from an accredited nursing program, must meet criteria for employment by Halifax Health
SKILLS, EXPERIENCE AND LICENSURE:
- Demonstrates good interpersonal, oral, and written communication skills
- Providing nursing care to pediatric, adolescent, and geriatric patients
- Works under the direction of a Registered Nurse
- Demonstrates ability to cope with stress, maturity and emotional stability
DUTIES AND RESPONSIBILTIES:
- Demonstrates knowledge of regulatory agency standards, nursing and organizational standards, Nursing Code of Ethics, and Nurse Practice Act by incorporating them into daily practice
- Performs all nursing duties, tasks and all requirements of hospice patients and family in a variety of settings. Reports changes to supervising RN
- Performs and documents in EMR (Electronic Medical Record) all required physical assessments with a high rate of accuracy/efficiency which reflect the patients current condition/Level of Care and all work is completed within agency guidelines
- Documents care provided per the comprehensive interdisciplinary plan of care to reflect patient and family needs
- Represents Halifax Health Hospice (HH-H) and utilizes effective communication methods and skills with all patients, family, facility staff, referral sources and coworkers
- Demonstrates a positive, informative and productive working relationship with all members of the IDG (Interdisciplinary Group) internal and external customers. Professional working relationships are maintained with hospitals and nursing homes. ALFs, Physicians' offices and all internal and external customers
- Demonstrates nursing procedure in accordance with current hospital, hospice and team standards, policies and procedures. Patient needs are met appropriately and in a timely manner.
- Demonstrates a knowledge of best practice in end-of-life care and support, pain and symptom management. Documentation in the medical record reflects appropriate interventions are used for optimal and ongoing pain and symptom management
- Maintains communication with the community physicians/ hospice medical team
- Identifies educational needs and provides appropriate training to patient/family.
- Works as assigned to assist in providing direct patient care 24 hours a day, 7 days a week including holidays
- Meets or Exceeds productivity standards for assigned position
- Maintains all company owned equipment in good working order
- Participates in orientation, supervision and education of new employees as assigned.
- Attends HH-H team, and General Staff Meetings or other End of Life In-services as required
- Meets Educational requirements for licensure and seeks appropriate education to enhance job performance
- Participates in QI activities as assigned
- Demonstrates professional development in the performance of their nursing practice
- Reports to work when scheduled (refer to Attendance Policy)
- Maintains a professional appearance that demonstrates a positive self-image (refer to Professional Appearance Expectations Policy)
- Maintain Patient confidentiality and professional boundaries (refer to Professional Boundaries Policy)
- Follows legal and ethical guidelines in providing nursing care
- Maintains competence and ongoing development of professional/clinical knowledge base through continuing education
- Maintains educational requirements for licensure and certifications required for specific clinical area
