Responsibilities of an LPNAC (Licensed Practical Nurse Assessment Coordinator):
- Assist with MDS (Minimum Data Set) assessments and documentation
- Ensure compliance with federal and state regulations
- Collaborate with nursing staff to gather resident data
- Maintain accurate medical records and reports
- Support care planning and coordination
Qualifications of an LPNAC (Licensed Practical Nurse Assessment Coordinator):
- LPN license in Pennsylvania (required)
- Experience with MDS assessments (preferred but not required)
- Strong organizational and communication skills
- Knowledge of long-term care regulations (preferred)
