Responsibilities Demonstrate positive customer service relations and community’s quality care and service standards. Incorporates wellness philosophy, standards and objective into the evaluation process for staff in the Wellness Department. Provides direct resident care/services and medication management when indicated. Completes and maintains individualized accurate health assessments and updated service plans for each resident. Provides direction as to format and approach to resident care/service management. Ensures community compliance with medication management procedures in accordance with state and company guidelines. Serves on Quality Improvement Committees that include Resident Care management, Infection Control and Safety. Serves as the Infection Control Coordinator following all OSHA safety guidelines. Graduate of an accredited School of Nursing. A minimum of two (2) years experience in a management position in Long Term Care, Assisted Living, or Skilled Nursing with an underlying knowledge in the overall disease processes of the elderly, as well as a familiarity in dealing with residents that have a diagnosis of Dementia. Demonstrated knowledge of nursing practices, techniques and methods as applied to resident care in the geriatric population found in assisted living and memory care. Solid understanding of the requirements for resident assessments and the care planning process. Ability to handle multiple differentiating priorities. Possess written and verbal skills for effective communication with residents, families, and health care team members. Experienced in organizational, time management skills. Demonstrates good judgment, problem solving and decision-making skills. Minimum of two (2) years of supervisory and management experience. Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications. Demonstrates knowledge of federal, state and local adult care regulations. Provides patient care based upon individualized plan of care as delegated by the Registered Nurse/Provider including patient's physical, emotional, cultural, spiritual, psychosocial, and educational needs. Assists provider with examinations, procedures and other activities related to patient care. Arranges for patient appointments for diagnostic testing, consults, referrals and admission to hospital. Monitors and maintains stock of medical supplies and proper functioning of equipment. Documents patient's vital statistics and other encounter data into patient chart and ensures that all required records are in the chart. Prepares exam rooms with appropriate instruments, supplies and equipment. Cleans exam rooms between exams and procedures. Receives and returns clinically related telephone calls and messages. Screens and forwards those requiring provider action. Completes medical/physical history forms and other related information for insurance companies. May administer oral and injectable medications as directed by a provider. Administers prescribed medications and treatments in accordance with approved nursing protocol. Serves as a role model and participates in educating and mentoring other members of the care team. Attends department and organization meetings. Completes annual and as needed compliance training. Practices safe, efficient use of supplies and resources. Maintains a clean, safe, and caring environment for patients, family and other staff. Assumes responsibility for one’s own professional development and continuing education. May serve as a preceptor, unit champion, or participate in shared governance. Adheres to all compliance, policies and procedures of the organization. Performs other duties as assigned. Supervisory/Management Responsibility This is a non-management job that will report to a supervisor, manager, director, or executive. Minimum Requirements Education - Completion of an LPN program recognized by the State Board of Nursing. Experience - No experience required. In Lieu Of In lieu of completion of a LPN program, will accept program equivalency recognized by the State Board of Nursing. Required Certifications, Registrations, Licenses Holds a current LPN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an LPN in the state the team member is working. If an LPN team member working in this position obtains RN licensure, Prisma Health will accept RN licensure for a limited time until the team member can be placed in an RN position.
