Position Summary:
We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our home care team. The ideal candidate will perform in-home patient visits, providing nursing care and education in collaboration with our interdisciplinary team.

Key Responsibilities:

  • Conduct home visits to provide skilled nursing care, including assessments, medication administration, wound care, and treatments.
  • Monitor and document patient conditions and progress.
  • Educate patients and families on managing medical conditions and promoting health.
  • Collaborate with the interdisciplinary team to ensure comprehensive and effective care plans.
  • Maintain accurate and timely electronic medical records (EMR).
  • Adhere to all safety protocols, infection control practices, and agency policies.

Qualifications:

  • Valid and active LPN license in New York.
  • Minimum of 1 year of nursing experience, preferably in home health, geriatrics, or acute care.
  • Current CPR certification.
  • Strong assessment, critical thinking, and communication skills.
  • Ability to work independently and travel within the service area (reliable transportation required).
  • Familiarity with EMR systems is a plus.
