The Role You'll Contribute
Responsible for providing and supervising direct and indirect total nursing care responsibilities to patients in the AdventHealth Hospital at Home (AH HaH) Program. This position utilizes the nursing process to achieve established patient goals. This role adheres to the AdventHealth Corporate Compliance Plan and all applicable local, state, and federal regulations. In addition, this position actively participates in outstanding customer service. Additionally, this role maintains respectful relationships with all individuals. Responsibilities for this role include assessing, planning, implementing, and evaluating patient care.

- Completes timely assessments including physiological, psychological, developmental, sociocultural, spiritual, and lifestyle factors, and makes appropriate decisions and interventions based on patients' health conditions.
- Sets measurable and achievable goals for patients, develops and implements individualized care plans, and modifies plans as needed.
- Assesses and interprets diagnostic data relative to patient age and condition, applying knowledge and skills to all diagnoses, procedures, medications, and equipment.
- Applies knowledge, skill, and adherence to regulatory standards in nursing practice, follows infection prevention precautions, and ensures medication safety.
- Educates patients on remote monitoring devices, virtual connection platforms, safety devices, communication tools, and resolves in-home safety issues.
- Fosters excellent nurse-patient relationships through effective communication with patients, families, and interdisciplinary teams, ensuring coordinated and integrated care.
The expertise and experiences you'll need to succeed:
- Associate's of Nursing
- 1 years of acute care nursing experience
- Registered Nurse State Licensure and/or Compact State Licensure In Florida
- Active American Heart Association BLS
- Valid in state Drivers License with current car insurance
- Bachelors degree in nursing
- Advanced Cardiac Life Support Cert (ACLS)
