What You'll Do

As our RN Clinical Manager, you'll play a key role in ensuring high-quality, compassionate home health care. Your leadership helps keep our nurses supported, our clients safe, and our mission thriving.

Collaborate on creating, reviewing, and updating patient plans of care
Provide nursing care according to physician orders and client needs
Document and report changes in patient condition
Educate and guide clients, families, and staff
Participate in QA programs, education sessions, and team meetings
Assist with assigning staff, reviewing referrals, and case coordination
Begin discharge planning from day one and complete discharge summaries
Conduct HHA supervisory visits and PRN visits as needed
Work rotating weekends and take on-call shifts when scheduled
Travel locally to patients’ homes and outreach locations
Maintain compliance with all Phoenix policies and regulatory standards
Support the DON in daily communication regarding patient care
Perform other leadership duties as assigned

What You Bring

Active Missouri or Compact RN License
1+ year of RN experience (Home Health preferred)
Valid driver’s license & current auto insurance
Current CPR certification
Ability to lift 50 lbs
Strong communication, time-management, and leadership skills
