Wound Nurse RN/LPN
Job Summary:
We are seeking a dedicated and experienced Wound Care Nurse to join our team. The Wound Nurse is responsible for assessing, planning, implementing, and evaluating the care of residents with wounds, ostomies, pressure injuries, and other skin conditions. This role ensures that all wound care practices meet current clinical standards and regulatory requirements.

Key Responsibilities:
Conduct regular skin assessments on residents to identify wounds, pressure injuries, or areas at risk.
Develop and implement individualized wound care plans in collaboration with the interdisciplinary team.
Perform wound treatments, dressing changes, and other therapeutic interventions as prescribed.
Monitor wound healing and document progress accurately and consistently.
Educate staff on wound prevention strategies, skin integrity, and proper dressing techniques.
Collaborate with physicians, dietitians, physical therapists, and other healthcare professionals to ensure comprehensive wound management.
Maintain compliance with state and federal regulations related to wound care in long-term care settings.
Participate in Quality Assurance and Performance Improvement (QAPI) programs.
Serve as a clinical resource and mentor for nursing staff regarding wound care practices.
Order and maintain wound care supplies and equipment.
Attend and contribute to care plan meetings and other interdisciplinary team meetings.
Qualifications:
Required: Active Virginia RN or LPN license
Preferred: Wound Care Certification
Experience: Minimum 1-2 years of clinical wound care experience, preferably in a long-term care or skilled nursing facility.
Strong knowledge of wound care products, protocols, and documentation practices.
Excellent communication, organizational, and critical thinking skills.
Ability to work independently and as part of a team in a fast-paced environment.
Physical Requirements:
Ability to stand and walk for extended periods.
Ability to lift and reposition residents when needed.
Manual dexterity to perform wound care tasks.
