Role Description
This is a part-time or PRN hybrid role for a Nurse Care Coordinator, located in Westborough, MA, with some work-from-home hours. The Nurse Care Coordinator will manage day-to-day care coordination as needed, new client intake meetings and creating detailed plans of care. Responsibilities include developing and updating care plans, communicating with healthcare professionals, clients, and family members, coordinating home care services, and providing exceptional customer service to ensure quality care is delivered. The role requires a focus on promoting health and well-being among older adults in various care settings.
Qualifications
Experience in Care Coordination and Case Management
Strong Communication skills, both written and verbal
Knowledge and experience in Home Care is a plus
Exceptional Customer Service skills
Registered Nurse (RN) licensure required
Ability to work effectively in a hybrid work environment
Ability to multitask and manage time effectively
Compassionate and empathetic approach to care
Pay and Benefits:
Competitive pay ($36.00-$45.00 an hour)
Pay may vary by level of experience and availability
