Description

 Astrana Health is seeking an NP/PA for our New Genesis Residential Care Program (RCP), a home-based primary care model serving complex, high-risk patients across San Diego. The role involves providing evidence-based, in-home care focused on improving outcomes, reducing hospitalizations, and minimizing unnecessary ER visits. The NP/PA will see an average of five patients per day, delivering comprehensive chronic and acute care in collaboration with a supervising physician, whether in the home, skilled nursing, or long-term care setting.

What You'll Do

  • Conducts face-to-face medical visits to patients in patient homes for routine visits, acute illness, health assessments and post-acute discharge for patients with complex medical conditions. (If telehealth flexibilities are extended, provider conducts video and telephonic visits for selected patients.) If needed as determined by leadership, they may be assigned to embedded clinic for period.
  • Completes a comprehensive health assessment to identify all active and chronic conditions, as well as determine all physical, mental and social needs present at the time of the visit.
  • Orders appropriate lab tests and diagnostic procedures and interpret results to aid in health promotion and accurate diagnosis and treatment of illnesses.
  • Performs medication review, prescribes medication or other forms of treatment per member’s needs and plan of care.
  • Facilitate goals of care discussions with patients and facilitates the completion of advance directives documentation.
  • Provides education on health conditions, medications, illness trajectory, and treatment options to patients, families, caregivers, and providers.
  • Determines effectiveness of the plan of care and reassesses and modifies the plan of care as necessary to achieve medical and health goals.
  • Must be comfortable performing in-home procedures, such as, not limited to wound care, IV/ IM medication, catheter insertion and removal.
  • Perform timely and accurate documentation of patients’ medical information and all activity related to patient care in electronic medical record system.
  • Appropriately assign the correct coding for acute and chronic conditions.
  • Complete full and appropriate documentation to reflect the services provided in accordance with company policies and procedures.
  • Communicates, by phone, in person, in writing, or electronically, with PCPs and Specialists to provide comprehensive care or to make referrals, as necessary. Consults with supervising physician as appropriate.
  • Participate in On-Call schedule which may include after-hours, weekend and holiday calls.
  • Provides feedback for program development. Participates in collaborative multidisciplinary team meetings to optimize clinical integration, efficiency, and effectiveness of service delivery as required.
  • Maintain patient confidentiality and uphold HIPAA regulations.
  • Other job duties assigned by Astrana/ RCP leadership.



Qualifications

  • Graduate of an accredited Master’s Degree in Nursing (MSN) program
  • One plus (1+) years of post-graduate experience working as a Nurse Practitioner in an Adult, Family, Internal Medicine, Geriatric, Palliative or Hospice setting preferred
  • Current, unrestricted RN/NP state license and current state driver's license
  • DEA License and furnishing privileges with the state board
  • Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)
  • Current BLS certification
  • Advanced knowledge of and experience with symptom management preferred
  • RN experience in Adult, Geriatric and or Palliative preferred



Environmental Job Requirements and Working Conditions

  • This is a hybrid position, where you will work predominantly out in the field in patient homes and also from home. Providers can expect an average of 5 in-home visits per day. 
  • Patient visits will take place throughout the San Diego area. 
  • The base pay range for this role is $70/hr - $82/hr. This salary range represents our national target range for this role.
  • This is a part-time, temporary position. We are seeking candidates who can commit to 2 days per week between M-F, 8 A - 5P. We can be flexible with hours and days. 
  • We are seeking candidates who can provide coverage from December 2025 through March 2026. There is opportunity to extend to per diem after the initial temporary period has ended.
