Job Type: Part-Time / Per Diem
Rate: $36/hour
Position Summary:
We are seeking a compassionate and reliable Registered Nurse (RN) to perform assessments within our Assisted Living community. The RN will be responsible for ensuring timely, accurate completion of assigned assessments, quarterly aide supervisions, and maintaining all required documentation in accordance with agency and DOH guidelines.
Duties and Responsibilities:
- Communicate proactively with facility administration prior to scheduled assessments to ensure all necessary paperwork and information are prepared.
- Maintain up-to-date knowledge regarding current admission assignments and protocols.
- Perform ALP (Assisted Living Program) assessments with flexibility and attention to detail.
- Maintain accurate and compliant client records following agency policy and procedures.
- Observe, assess, and report any changes or adverse signs and symptoms in residents’ conditions.
- Maintain strict confidentiality of resident information at all times.
- Participate in chart audits, peer reviews, and case conferences with clinical staff as needed.
- Obtain additional certifications and participate in ongoing training as requested (such as OASIS training, PRI/Screen Certification, or QAS analyzer use).
Qualifications:
- Current Registered Nurse (RN) license in New York State (required)
- Prior experience in assisted living, home care, or long-term care preferred
- Strong clinical assessment and documentation skills
- Excellent communication and interpersonal abilities
- Ability to work independently while maintaining professionalism and compassion
