Why This Role Matters


As an RN Field Clinical Manager, you’ll be a key advocate in our Integrated Care Model—helping members access the right services at the right time and improving health outcomes across Central Upstate NY Region. 🏙️


What You’ll Do


  • Assess, plan, and implement individualized care strategies
  • Perform NYS UAS Assessments in members’ homes twice per year
  • Coordinate referrals for social services and community resources
  • Serve as a single point of contact for care plan management
  • Advocate for members and families to ensure their needs are met


What We’re Looking For


  • Current, unrestricted RN license in NY
  • 2+ years of clinical experience
  • 1+ year in community case management
  • Ability to travel within Central Upstate NY Region


Preferred:


  • Behavioral health experience
  • Familiarity with UASNY and electronic charting


💵 Pay Transparency


Hourly pay ranges $28.27–$50.48, based on experience and location, plus eligibility for bonuses and a full suite of benefits designed to support you. Pay is determined by factors including local labor markets, education, and certifications.
