Position Overview:

As a Home Health Registered Nurse, you’ll provide in-home skilled nursing care to patients by developing personalized care plans, administering medications and supporting recovery and independence. Whether you're experienced in home health or coming from an acute care setting, this role offers a rewarding opportunity to apply your clinical expertise in a more independent, patient-focused environment.

What You’ll Do:

    • Provide skilled nursing care, assessments, and interventions in the home

    • Serve as case manager and patient advocate

    • Collaborate with interdisciplinary care teams

    • Educate patients and families on care plans and self-management

    Minimum Qualifications:

    • Active RN license in Michigan

    • 2+ years of clinical nursing experience (Home Health preferred)

    • Epic or Home Care EMR Experience

    • Strong communication, assessment, and organizational skills

    • Compassionate, dependable, and mission-minded

    • Valid driver’s license and reliable transportation required

There are over 20 care spaces to treat patients with minor to major medical and traumatic emergencies. Gerber ED has over 27,000 visits/year and ranks highly in patient experience and throughput measures. We value teamwork, culture, and process improvement from each member of our teams! About the unit Gerber ICU is 11 beds, 2 of those are observation beds, 9 are inpatient beds. The ICU is going to be refreshed with new paint, flooring, and artwork.
