LPN Charge Nurse

Schedule: 12pm to 8am. 5 nights a week. Rotating Weekends. 
or
Schedule: 8am to 4pm. 5 days a week. Rotating Weekends. 

Berkshire Nursing Home & Rehab Center is a 175 -bed skilled nursing and rehabilitation facility located in West Babylon, NY. Our mission is to provide the highest level of care for each of our residents. Currently, we are searching for LPN Charge Nurses. 

Job responsibilities include but are not limited to.

• Ensures proper resident care is maintained in a clean and safe environment.
• Makes nursing diagnoses, which serve as the basis for the strategy of care
• Collaborates with the resident, members of the healthcare team
• Records the administration of medication or any change in patient condition
• Monitor assigned staff to ensure established safety regulations are followed and that all tasks are performed following all state and government regulations and safety procedures as outlined by the facility
• Follows procedures related to the use of nursing equipment and supplies
• Performs other duties as directed.

Requirements:

· Experience in long term care.
· Currently licensed by the State of New York. 
· Possesses leadership and supervisory qualities.
· Possesses assessment skills that enable the recognition of condition changes in the resident.
· Ability to coordinate all functions in the operation of a resident unit through effective team nursing assignments.
· Maintain meaningful and accurate documentation of services provided.
· Prior computer skills in (Excel, Word, and Microsoft Office) a plus.
· Must have work authorization.
