We are a well-established 99-bed Skilled Nursing Facility in Glendale committed to delivering exceptional care and maintaining high clinical and regulatory standards. We are seeking an experienced QA Nurse / RN Supervisor who can thrive in a hybrid role balancing both Quality Assurance and Nursing Supervision.

Position Summary

This role is 50/50 hybrid between Quality Assurance and RN Supervisor duties.

  • 2–3 days per week: Quality Assurance (QA)

  • 2–3 days per week: RN Supervisor

The ideal candidate has strong SNF experience, is highly organized, and understands both clinical operations and compliance requirements.

Responsibilities

Quality Assurance (2–3 days/week)

  • Conduct QA audits and chart reviews

  • Ensure regulatory compliance with Title 22 & federal guidelines

  • Maintain survey readiness and support plan-of-correction follow-through

  • Oversee infection control and QAPI initiatives

  • Review incidents and conduct root cause analyses

  • Assist with staff education and training on compliance and documentation

RN Supervisor Duties (2–3 days/week)

  • Oversee nursing staff and daily clinical operations

  • Support resident care and respond to clinical issues

  • Participate in care planning and IDT meetings

  • Assist with admissions, assessments, and follow-up documentation

  • Provide leadership, coaching, and support to LVNs/CNAs

  • Ensure policies, safety practices, and standards of care are upheld

Requirements

  • Active California RN license (Required)

  • Skilled Nursing Facility experience (Required)

  • Experience in QA, infection control, or RN supervision

  • Strong knowledge of SNF regulations, survey processes, and Title 22

  • Excellent communication, leadership, and documentation skills

  • Ability to train and educate staff
