Job Objective:

Collaborates with department leadership and staff, as well as 24/7 donors/family, physicians, specialists and ancillary staff in the planning, organizing, coordinating, and executing of specified 24/7 Recognition program activities. Areas of practice are highly multidisciplinary and multi-faceted; timely, open, frequent, and closed loop communication are consistently required to meet 24/7 donor needs. The 24/7 LVN serves in an adjunct, supportive role to Registered Nurses within the 24/7 Recognition program, to ensure efficient access to a variety of health care programs and services in gratitude for donor contributions, which are crucial to the financial viability of Eisenhower Health.

Essential Responsibilities

1. Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations. 2. Assists with the coordination of safe, high-quality patient care; integrates services and prescribed treatment among the hospital system’s different programs, service sites, departments and physicians in a manner that is valued and acceptable to all stake holders, especially 24/7 members. 3. Tightly collaborates with 24/7 members and/or their designee(s) the coordination and scheduling of all medically related appointments across the healthcare continuum. 4. Facilitates 24/7 member’s participation in all decisions concerning care and appropriately advocates on their behalf 5. Adheres to health-system scripting; seeks assistance if unclear. 6. Recognizes appropriate and opportune moments to capture/relay vital information to 24/7 leadership to support philanthropic activities and support member retention. 7. Ensures physician orders to initiate treatments and scheduling are available and viable; judiciously intervenes/connects with physicians and 24/7 team to rectify issues. 8. Intervenes appropriately to reduce anxiety and enhance patients’ ability to follow-through with appointments and prescribed care. 9. Involves 24/7 Registered Nurses and leadership to provide up to date medical information on diagnostic testing and planned treatments; wisely recognizes when a members’ need for information/support is outside LVN scope and/or personal knowledge, and facilitates or refers them for timely contact with an appropriate entity. 10. Escorts 24/7 patients to and from various departments for tests and treatments; assesses need for adaptive equipment, wheelchairs, etc. to support safety. Remains with patients as requested and pro-actively orchestrates or takes additional customer service measures and actions as appropriate, to ensure a high-quality patient experience. 11. Recognizes potentially critical situations and takes action to minimize adverse outcomes. 12. Conveys through action and communication, an appreciation for the central role 24/7 donors play in the fiscal health of the organization and actively promotes positive perceptions of the program. 13. Shares knowledge and acts as a professional resource to colleagues and other members of the healthcare team. Trains, orients, coaches, and positively supports new hires to ensure confident, seamless integration into the department and boosts new team members’ ability to uphold safety, care quality, and outstanding customer service. 14. Embraces change and continuously demonstrates a commitment to utilize evidence-based practice. 15. Actively seeks opportunities to support/up-lift department morale. 16. Earnestly promotes a positive department image when interfacing with health system staff/ professionals, and either personally takes action to address issues and/or immediately informs and engages 24/7 leadership 17. Involves appropriate line managers in service recovery as necessary. 18. Provides patients with information on how to contact 24/7 staff. 19. Treats all patients, staff, physicians, volunteers, and visitors with kindness, empathy, dignity, and respect. 20. Eagerly and routinely employs critical thinking, and/or collaborates with 24/7 leadership/peers/staff to improve/streamline workflow processes to ensure and uphold high-caliber service and conserve valuable health system resources. 21.
