Responsibilities:

Coordinate and provide direct patient care to residents in accordance with their individualized care plans
Assess, plan, implement, and evaluate the effectiveness of patient care programs and services
Develop and maintain positive relationships with residents, families, and staff
Collaborate with interdisciplinary teams to ensure seamless continuity of care
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Provide educational support and training to staff on patient care and departmental policies and procedures

Requirements:

Current licensure as a Registered Nurse (RN)
Experience as a Regional Nurse or in a similar role
*Strong communication and interpersonal skills
*Ability to work effectively in a fast-paced environment
