Responsibilities

The Psychiatric/Mental Health RN is responsible for managing the care of patients ages 18+ y/o experiencing psychiatric/mental health disorders. Care management for these patients would require psychosocial, physical, and behavioral assessments related to specific conditions and providing individualized interventions based on these assessments. The Psychiatric/Mental Health RN is responsible to assess, plan, implement, evaluate, and supervise nursing care of patients within the area of assignment pursuant to the policies and procedures of Prairie Care. Requires knowledge of nursing practices, which can be gained only through an accredited school of nursing.  Oriented and educated in gender, cultural, and age appropriate care for the assigned unit.

 

Responsibilities and Accountabilities:

  • Assesses and monitors young adult patients’ daily functioning and assists in mental health symptom management. Demonstrates purposeful, informed, results oriented critical thinking resulting in safe, quality care and good nursing judgment. Monitors own level of expertise continuously using ongoing self-assessments. Collaborates with team, seeks timely advice, and gives and always accepts feedback to ensure safe patient care
  • Effectively uses therapeutic approaches to manage milieus with a focus on reducing the use of restraint and seclusion when possible
  • Plans and facilitates nursing groups with patients when scheduled during shift
  • Responsible for nursing assessment upon admission and the necessary discharge planning required in the partial hospital program
  • Tracks and communicates pertinent medical and psychiatric information both verbally and through written documentation in EHR. Documents patient assessment findings, behavioral and psychosocial responses to nursing interventions, and progress towards problem resolution.
  • Administers medications in an accurate manner consistent with policy and good nursing practice including verifying allergies and/or contraindications and monitoring for side effects. Completes medication refills when needed. Educates patients and families about medications.
  • Performs appropriate interventions as ordered by physician in an accurate and timely manner including needed testing such as lab draws, EKG, UDS, Genesight, and HcG testing. Documents results and provides safe handoffs to providers about the results.
  • Knowledgeable of and performs safe infection control practices including good hand hygiene, standard precautions, cleaning equipment and the environment, reporting and managing of suspected infectious/contagious diseases, and teaches patients’ practices such as washing their hands and covering their cough.
  • Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s developmental age and to the patient’s and family’s diversity, preserving their autonomy, dignity, and rights.
  • Maintains confidentiality in matters related to patients and families.
  • Maintains current competency in child/adolescent psychiatric/mental health nursing including the safe use of restraints and seclusion and effective de-escalation of patients using collaborative problem solving, therapeutic self, relationship-based care, and trauma informed practice

Qualifications

  • Graduate of an accredited Nursing School, Nursing Degree required.  BSN Preferred. Current licensure as an RN with the Minnesota State Board of Nursing is required.
  • Evidence of 1 year of psychiatric/mental health nursing experience within the past two years preferred
  • Additionally, the RN may have professional certifications, including but not limited to an RN ANCC Certification in Mental Health Nursing, Pediatric Nursing, and/or Holistic Health
  • Must have critical thinking skills and ability to problem solve
  • Ability to communicate and make safe judgment calls in stressful situations
  • Ability to multi-task, use time efficiently, and prioritize appropriately
  • Ability to communicate in a professional manner in both verbal and written format and model a positive teamwork attitude
  • Ability to intervene in pre-crisis or crisis situations by utilizing de-escalation techniques, limit setting, and the use of least restrictive interventions
  • Interpersonal skills—the individual maintains confidentiality, remains open to others’ ideas and exhibits willingness to try new things.
  • Must have basic computer skills using Microsoft Office products and EHR
  • Knowledgeable of the Joint Commission standards for restraint/seclusion use and documentation requirements.
  • Ability to react quickly and calmly as part of a team to handle volatile and potentially emergent situations
  • Quality management—the individual looks for ways to improve and promote quality and demonstrates accuracy and thoroughness.

Compensation for this role ranges from $38 - $ 51 per hour  depending on education and experience.

Duties and Responsibilities: · Conducts home assessments (and virtual assessments, as needed) for prospective members and re-assessments of current members using the Uniform Assessment System (UAS); make enrollment and continued eligibility determinations by utilizing clinical expertise and critical thinking skills. As needed, conducts assessments in settings other than home as needed, e.g. skilled nursing facilities or virtually · Navigates multiple technology//digital platforms such as UAS software, electronic medical records, TruCare, etc., to conduct and document comprehensive assessments accurately and efficiently · Completes assessment of service needs per CMT and member's family//caregiver’s request to determine service needs or any necessary adjustments · Reviews previous nursing assessments//medical notes to determine necessary adjustments//updates in Care Plan and//or to assist with nursing home admission · Conducts in-person Care Management visits in support of person-centered service planning · Liaisons between the member and the CMT; assesses home environment and psychosocial status · Provides feedback to home care agencies and CMT regarding home care issues and contract nurses and aides performance · Provides grass roots, community-based training for frail population care including self-care techniques and prevention strategies · Ensures (prospective) members and significant others//responsible parties understand and are in agreement with enrollment in a managed long-term care plan · Additional duties as assigned Minimum Qualifications: · New York State RN license · Ability to travel around downstate New York which includes the 5 boroughs, Long Island, Rockland, and Westchester. · Prior work experience requiring technological proficiency including the ability to navigate multiple technology platforms and modalities · Prior work experience requiring intermediate Microsoft Word, Excel, and Outlook skills · Prior work experience requiring time management, critical//creative thinking, communication, and problem-solving skills · For PEDS positions only: 1 year of pediatric clinical field experience and//or experience with families and child serving systems, including child welfare and//or medically fragile//developmentally disabled populations Preferred Qualifications: · Work experience using electronic patient health information (PHI) database usage especially UAS · Previous field-based experience assessing, planning, and evaluating member's care by making home or facility visits for intake//reassessments or start of care (SOC) Community Home Health Agency (CHHA) visits · Work experience preferred in one or more of the following areas, geriatrics, home care, discharge planning, case management, and//or medical surgical nursing · Knowledge of health insurance, Medicaid, Medicare and MLTCP · Experience working with a frail adult or elderly population with the ability to determine appropriate care plans and services for frail population as well as negotiate initial service plans so that members and families are in agreement · Language preferences - Spanish, Russian, French, Creole, Mandarin, Cantonese · Demonstrated ability handling heavy caseloads · NY state driver’s license Hiring Range*: Greater New York City Area (NY, NJ, CT residents): $102,000 - $120,000 All Other Locations (within approved locations): $71,600 - $106,505 $10,000 Sign-On Bonus As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision.
