The Heart of the Role: Your Impact
As the Resident Care Coordinator, you are the link between clinical needs, effective communication, and regulatory accuracy.

Care Plan Support: Assist in the development, implementation, and daily monitoring of individualized care plans for residents, ensuring they are accurate, current, and followed by all staff.

Clinical Support: Assist the nursing team by ensuring timely and accurate documentation, follow-through on physician orders, and consistent communication regarding resident status changes.

Documentation and Compliance: Ensure all resident care documentation and daily charting meet federal, state, and facility standards. This role supports the integrity of our records and adherence to compliance requirements.

Interdisciplinary Team Member: Facilitate effective communication and coordination among nurses, physicians, CNAs, therapists, families, and outside healthcare providers.

Communication Liaison: Serve as a key clinical point of contact for residents and their families for routine updates and information, addressing concerns with empathy and clarity.
What We Need You to Bring (The Essentials)

Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania (Mandatory).

Minimum of 2-3 years of clinical experience in a long-term care, skilled nursing, or assisted living environment.

Proven experience with nursing documentation, charting, and assisting with resident care planning.

Exceptional organizational skills and the ability to manage multiple priorities in a fast-paced setting.

Strong verbal and written communication skills—you'll be communicating with RNs, CNAs, and concerned family members.
