This is a statewide position and will report to their nearest DHW location.

This position may be eligible for limited telework after successful completion of probation and necessary training. Approval to telecommute is not guaranteed and subject to termination at any time.

EXAMPLE OF DUTIES:

  • Conduct in-home level of care assessments to determine eligibility for Personal Care Services (PCS), Aged & Disabled (A&D) Waiver programs.
  • Observe care delivery and conduct interviews to ensure services meet state and federal standards.
  • Explain available services, and complete comprehensive evaluations to help individuals access Home and Community-Based Services (HCBS).
  • Serve as a trusted resource for participants, families, and providers.
  • Answer questions about HCBS and supplemental services, assist with provider-related concerns, clarifying service authorizations and eligibility. 
  • Support the team and maintain program integrity through documentation and correspondence. 
  • Participation in staff meetings and general office duties that keep operations running smoothly.
  • Conducting regulatory compliance reviews through client sampling.
  • Assess clients' health , emotional, and rehabilitative needs. 
  • Applying critical thinking and clinical judgement to evaluate care quality, writing clear, concise, and legally sound reports.

Observes, records and reports patients treatment, care, symptoms, physical and mental condition, behavior and progress. Administers prescribed medications including intramuscular injections and observes for adverse reactions. Performs treatments including change of sterile dressings, insertion of nasal gastric tubes, catheterizations and removal of sutures. Interviews patients to obtain health information and instructs and counsels patients regarding basic health needs. Teaches patients and their families the importance of proper diet, proper positioning, range-of-motion exercises, transfer techniques and personal hygiene procedures. Charts administered medications, symptoms, accidents, unusual behavior, doctors' visits and observed side effects of drugs. Orders lab and x-ray work under physicians orders.
