Be the Best Hospice Nurse Practitioner You Can Be 

If you meet these qualifications, we would love to meet you: 

Graduated from an approved school of professional nursing - Masters degree
Current and unecumbered advanced practice nurse license in the state(s) practicing

One year of experience as an NP

Certified by a nationally recognized advanced nurse practice organization

Possess and maintain valid CPR certification while employed in a clinical role (preferred)

Must be a licensed driver who can travel to all business locations

Meet the regulations and requirements of the state(s) in which the program provides services

Responsibilities:  

As a Hospice Nurse Practitioner, you will:

Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
Monitor patient progress; update the Team Director and team members; consult regarding discharge planning and patient/family teaching

Maintain continuity of care in coordinating with the care team and case manager for patients with ongoing needs
Prescribe medication as permitted by the state and/or the state Board of Nursing

Pre-Hospice

Face-to-face clinical and needs assesment at intake, preferably in the patient's home setting, inclusive of patient's medical, emotional, spiritual, social, and practical needs
Face-to-face reasssessment done when needed and at least every 60 days. Plan of care development based on these assessments and incorporting the patient's and family's priorities
Initiate communication with attending physcians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
Clinical supervision of the care plan including all medicine orders
Collaborate with the rest of the pre-hospice team including alerting the team to changes and newly identified patient needs as they arise

We are hiring a per diem Nurse Practitioner to join our care team with Athena Hospice of Rhode Island! This is to assist with on-call admissions visits. The nurse practitioner provides comprehensive hospice services and care through the evaluation and assessment of the health history, health promotion, diagnosis of medical conditions, management of health problems, prescribing medications or treatments, and other duties. Experience & Education: Graduate from an accredited school or nursing and accredited nursing practitioner program. Active and unencumbered Registered Nurse license in Rhode Island. CPR certified. DEA licensed. Master's degree with a minimum of one (1) year experience as an NP. One (1) year of hospice or palliative care experience. Accreditation from an approved certifying body for advanced practice nursing. Preferred certification in a specialist area such as hospice or pediatrics. Duties & Responsibilities: Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering, and interpreting results from lab and diagnostic tests or procedures, diagnosing health conditions and documenting the findings in the patient's medical records. Manages health conditions and terminal illnesses by developing plans of care, prescribing medications and/or treatments, obtaining consultation or making referrals and coordinating hospice care services. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. Reviews clinical information, and coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. Completes, maintains, and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. Ensures compliance with the Medicare conditions of participation and other state regulations governing the provision of healthcare. Builds from the resources in the community to plan and direct services to meet the needs of individuals and families.
