Responsibilities

Communicate significant findings, problems, changes in condition or environment to the clinical manager/attending physician and other members of the interdisciplinary team.
Complete face to face encounters and assesses the following: disease related changes, decline in function, adherence and effectiveness of pain and symptom regimen, the patient and family coping mechanisms and quality of life.
Order and review appropriate lab, x-rays, EKG's and other diagnostic procedures for palliation and treatment of the terminal disease as dictated by the patient's condition and documents the treatment plan accordingly and reviews results with collaborating physician.
Make referrals to other disciplines as indicated by the patient's needs.
Actively participate in interdisciplinary team conferences.

Qualifications

Current and unencumbered license as a Nurse Practitioner in the state the employee is assigned to work by the Company.
Minimum of one (1) year experience as a Nurse Practitioner or minimum of one (1) year experience as a hospice Registered Nurse in last three (3) years.
Must have the authority to practice as a nurse practitioner in the state where services are provided.
Must have the authority to prescribe as a nurse practitioner in the state where services are provided.
Must have obtained Medicare billing privileges as a nurse practitioner.
Must meet physician-nurse practitioner restrictive collaborative requirements specific to state assigned to work by the Company and supply a copy of all active collaborating physician agreements and indicate whether these will be transferable to your employment with Amedisys.
Must have insurance required by state law for nurse practitioners to practice [and prescribe] in the state where services are provided or be eligible for such insurance requirements to be satisfied by group coverage with an employer.
Knowledge of Medicare reimbursement and coding for all levels of service - home, office, nursing home, assisted living, etc., Medicaid and other regulatory requirements.
Current CPR certification.

The nurse practitioner builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. 2. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record. 3. Manages health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and / or treatments, obtaining consultations or making referrals and coordinating hospice care services. 4. Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification. 5. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. 6. Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. 7. Assures that patients receive appropriate measures to control symptoms, through collaboration with interdisciplinary team members. 8. Reviews and develops protocols for treatment and proposes options for interventions based on the literature in collaboration with the collaborating physician. 9. Consults the collaborating physician when the patient's plan of care is outside standardized practice and protocols. Communication 1. Communicates and collaborates with the interdisciplinary group to create, review and revise the patient's plan of care. 2. Consults with the patient's attending physician, the hospice medical director and other healthcare practitioners regarding the patient's eligibility for hospice care, ongoing care needs and medical management related to the patient's terminal condition. 3. Communicates with other community health practitioners to coordinate the plan of care. 4. Attends and participates in hospice interdisciplinary group meetings. 5. Completes, maintains and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. 6. Educates/counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients. 7. Works in cooperation with the family/caregiver and hospice interdisciplinary group to identify the goals of care and meet the care needs of the patient and family/caregiver. 10. Our expert clinicians function as a collaborative team, incorporating all elements of inpatient and outpatient plastic surgical care at our main hospital. The team currently consists of two pediatric plastic surgeons and two nurse practitioners. The physician assistant joining our team will function to: Provide first operative assistance for Burn and Plastic and Reconstructive surgical procedures in the operating room, including cleft and craniofacial care, general plastic surgical procedures, and acute and secondary burn surgery. Perform a comprehensive admission history and physical on patients admitted to the Burn Service or Plastic and Reconstructive Surgery Service and document in record as appropriate. Care for inpatients in the Burn Unit, Pediatric ICU, Medical-Surgical floors, and Emergency Department, including daily team rounds, development and implementation of care plans with attendings based on ongoing clinical assessment and collaboration with the multidisciplinary team, and discharge planning. Perform burn and other wound care, including hydrotherapy, debridement, dressing changes.
