Your Role on the Team Position Summary: The Nurse Coordinator functions independently to coordinate care of oncological patients. Responsible for planning, education and support of oncology patient and family. Assures that the continuum of care process is complete, including intra-interdepartmental process to include: pre-admit, hospitalization, discharge and follow up in the OP. Maintains program data for analysis and RFPs. Works with Director and Medical Director / team for the development and growth of the program. Provides nursing services as needed.
