Summary:

The Infusion RN works closely with the interdisciplinary health care team to facilitate coordination of care across the continuum. The Nurse focuses on expanding the knowledge and skills necessary to provide individualized patient care based on physical, psychosocial, cultural, educational, safety and age appropriate consideration of assigned patients. The Nurse will also use advanced clinical skills to perform health assessments relating to the chemotherapy plan and triages patients according to assessments that include such areas as emotional response, physical status, acuity, response to treatment, potential complications and educational needs of the patient and their family. The Nurse actively seeks and provides feedback for improved clinical practice, and demonstrates how nursing practice impacts the organizational vision, mission, goals and the care delivery system. The nurse complies with all regulations and standards of regulatory and accrediting bodies.

Responsibilities:

Collects and analyzes significant data to assure the development of an integrated plan of nursing care for patients with complex oncologic problems.

Identifies goals for patient care pertinent to patient populations with specialized needs and problems.

Independently coordinates and implements plan of care through a prioritized series of complex nursing interventions.

Analyzes the relationship between patient problems, intended outcomes and actual outcomes.

Evaluates and integrates best current evidence with clinical expertise in the delivery of patient care

Utilizes appropriate information and technology to communicate, collaborate with the interprofessional team, mitigate error and support decision making across the continuum.

Effectively engages in teamwork and cooperation in an effort to provide safe, quality outcomes for patients.

Basic Cardiac Life Support (Required)

Day shift: 8, 10, and 12 hour shifts available 1 weekend every 4 weeks Involved in your own community Maintain PIV skills Not losing any acute care skillsets Obtain certifications and pilot projects No case loads Hands on 1:1 direct care Home Health Focused Competencies/Visit Type •Oasis SOC ( with office support) •Wound Care •Wound Vacs •Foley’s/ Irrigations •Pleurex, Surgical drains, & drain removal •PEG tubes and enteral feeds •Ostomies in extenuating circumstances Infusion Focused Competencies •PICC/ Port / Midline Care •Picc and Midline removals •IV antibiotics •TPN •Chemo disconnects •PCA management •Hospital Hook ups •Milrinone •Diuretics and Steroids •Hydration •Anaphylaxis Policy •IVIG/ Biologic infusions/ relief •Peripheral IV insertion and removal •Pump competency Responsibilities : ·These nurses are experts in administering medications, fluids, and other treatments directly into the patient’s bloodstream through intravenous (IV) lines. · Handling various types of infusions, including antibiotics, pain management, chemotherapy, nutrition, and factor therapy. ·Unlike hospital-based care, these nurses bring their expertise to patients’ homes. ·Assess patients, monitor their progress, and ensure safe and effective administration of infusions. ·May simultaneously administer two different infusions to a patient. This could involve managing multiple IV lines, coordinating medications, and ensuring compatibility and safety. ·Work closely with physicians and other healthcare providers. ·Following established protocols and collaborating on treatment plans to meet each patient’s unique needs. ·Educate patients and their families about infusion procedures, potential side effects, and self-care. ·Document vital signs, infusion rates, adverse reactions, and any adjustments made during the process. Some patients may have CVADs, such as PICC lines or implanted ports. · Skilled in maintaining and using these devices for safe and efficient infusion delivery . Qualifications: Clinical Nurse-NCL (Non-Clinical Ladder) track: requirements: ·Diploma or Associate Degree in Nursing (ASN) from an accredited school of nursing ·15+ months of relevant professional nursing experience ·Pennsylvania (or New Jersey) RN licensure (depending on hire location) ·BLS/CPR as a healthcare provider, as per the American Heart Association ·Have a car, valid driver’s license, and car insurance OR Clinical Nurse 2 track requirements: ·Bachelor of Science in Nursing (BSN) from an accredited school of nursing ·15+ months of relevant professional nursing experience ·Pennsylvania (or New Jersey) RN licensure ·BLS/CPR as a healthcare provider, as per the American Heart Association ·Have a car, valid driver’s license, and car insurance.
