LPN Position Summary

As a Licensed Practical Nurse (LPN), you will play a vital role in supporting the health, safety, and dignity of our residents in an assisted living environment. You will provide hands-on nursing care, coordinate treatments, and collaborate with caregivers and leadership to ensure high-quality resident outcomes.

Key responsibilities include:

  • Resident assessments and care planning

  • Medication administration and treatment management

  • Monitoring and documenting changes in resident condition

  • Accurate clinical documentation

  • Supporting resident independence while maintaining dignity

  • Collaborating with caregivers, families, and healthcare partners

Qualifications & Requirements

  • Active State of Connecticut LPN License (required)

  • Prior experience in assisted living, senior living, long-term care, or skilled nursing preferred
