LPN Float Pool Position Summary: Pick your own shifts at various locations based on the needs of Monarch Healthcare Management! This is a full-time or on-call position with weekend requirements. The LPN participates in the development and modification of the resident Plan of Care, implementing interventions that are delegated, ordered, or prescribed by the RN or physician. As an LPN you are providing residents with the highest quality of direct routine care, under the supervision of the RN and in accordance with the Minnesota BON standards, the Resident’s Bill of Rights and the facility Quality Assurance and Performance Improvement (QAPI) program. The Monarch Float Pool was created to help offer staffing assistance to our own Monarch facilities. Pick your own shifts based on the needs of Monarch Healthcare Management. Travel is expected for this position and there are benefits available. Qualifications: Completion of a professional nursing education program approved by the Minnesota Board of Nursing. Current Minnesota License as a Practical Nurse. Current Certification in Cardiopulmonary Resuscitation (CPR) and emergency care or be willing to get one upon hire. Experience in skilled nursing and/or assisted living facilities. Overview A licensed practical nurse is a clinically competent position that provides care to a group of home health patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members to assure resident safety. Responsibilities Perform initial data collection on all patients/residents and follows up on data collection as per policy. Participate in implementing person driven plans of care as developed by the Registered Nurse within established policies and procedures and continually works to meet individual patient/resident needs. Demonstrate knowledge of medications and their correct administration based on age of the patient/resident and their clinical condition. Observe, document and report patient/resident responses to medications. Follow patient/resident, family and caregiver education plan of care as developed by the Registered Nurse, clearly communicates information and documents patient/resident response to education. Document according to current standards and policies. Plan for patient/resident discharge, collaborates with the Registered Nurse and discharge planners for long and short term patient/resident needs. Coordinate and direct patient/resident care appropriately. Perform charge nurse or building responsibilities when assigned. Utilize the principles of Relationship Based Care while performing duties.
