What You’ll Do:

The Nurse Manager is a key leader within the multidisciplinary team, overseeing the healthcare needs of individuals in a community setting.  

Clinical Care & Treatment

  • Assess health needs, develop care plans, and administer medications/treatments per regulations (DPH, CARF).
  • Provide direct support, including ADLs, mobility assistance, and community integration.

Care Coordination & Advocacy

  • Liaise with physicians, specialists, and stakeholders to ensure seamless healthcare delivery.
  • Coordinate hospital admissions, discharges, and follow-up care.

Compliance & Safety

  • Maintain medication logs, infection control, and safety protocols.
  • Ensure program adherence to funder, state, and accreditation standards.

Stakeholder Communication

  • Deliver timely health updates to families, guardians, and funders.
  • Document and report care outcomes accurately.

Staff Leadership & Training

  • Train and mentor staff on medical protocols and person-centered practices.
  • Foster a supportive environment using Positive Behavior Supports (PBS).

Operational Flexibility

  • Participate in on-call rotation (24/7) and provide holiday/weekend coverage as needed.
  • Assist with day/residential program coverage during staffing gaps.

What You Bring:

  • Experience: Supporting mental health needs (personal or professional).
