Job Description

 

We are looking for a devoted and caring Home Health Aid to provide top notch care for our clients.

 

Some responsibilities include:

 

  • Provide assistance in personal care needs such as bathing, dressing, grooming etc.

  • Assist or provide light housekeeping work

  • Engage patient in enjoyable activities such as games, books, art etc.

  • Provide companionship, empathy and basic emotional support

  • Provide and assist with meal preparations, grocery shopping, laundry etc.

  • Assist with medication reminders

  • Ensure patient confidentiality

Pay

Certification

  • HHA or PCA required (Certified in NY)

Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly Documentation. Accurately and completely document client care on activity sheet Compliance.
