As a Hospice CNA You will:  

Personal Care: Help with bathing, dressing, toileting, mobility, and repositioning

Monitor & Report: Observe and report changes in the patient’s condition

Mobility: Assist with ambulation, transfers, ROM exercises and safe equipment use

Medication Assistance: Help with self-administered medications per state regulations

Emotional Support: Offer companionship and comfort to patients and families.

Light Housekeeping: Change linens, assist with meals, and maintain a clean space.

Documentation: Maintain accurate and timely documentation

Team Participation: Collaborate with hospice interdisciplinary team

Qualifications

Meets the training requirements of the State

Completed a minimum of 75 hours of classroom and supervised practical training 16 hours of classroom must be completed prior to a mandatory 16 hours of practical training

Minimum of six months experience including providing personal care

At least 18 years of age

Ability to read and follow written instructions and document care given

Understands hospice philosophy, comfortable providing care to the terminally ill

Satisfactory references from previous employers

Ability to work with little direct supervision

Secure with issues of death/dying

Provides a calm manner when in a patient’s home

Possess and maintains current CPR certification

Demonstrates tact, patience, and good personal hygiene

Successfully completes a probationary period

This probationary period is to ensure the aide is competent in the above qualifications and is documented by the supervisor and the employee

Strong oral and written communication skills

Good organizational skills

How you make a difference The Certified Nursing Assistant performs basic and specialized nursing care functions under the supervision of licensed nurses. They adhere to local and regulatory policies and procedures to ensure proper patient care. The Nursing Assistant collaborates with other healthcare team members and correctional officers to plan and implement patient care activities in a correctional setting. After a general orientation to medical-surgical and psychiatric patient care areas, they may also float between units to ensure adequate nursing staff for patient care areas. Key Responsibilities Provides basic nursing care to patients under the direction of a licensed nurse, including assistance with ADLs, vital sign measurement, and accurate documentation of patient data. Maintains professional behavior and prioritizes work flow to ensure completion of all assigned duties, while adhering to policies and procedures and promoting patient safety and confidentiality. Collaborates with other members of the health care team, participates in staff meetings and training sessions, and notifies the licensed nurse of any patient concerns or issues.
