LPN Care Coordinator Position Summary: The LPN Care Coordinator supports the Director of Nursing, Assistant Director of Nursing, and RN Nurse Manager in leading and managing a specific nursing unit by assisting with care planning, resource evaluation, and implementation of the nursing process. This role helps collect resident data, contributes to assessments such as MDS and RAPS, develops and updates comprehensive care plans, and collaborates with the interdisciplinary team to ensure resident needs are met. The LPN Care Coordinator monitors resident conditions, reports changes to RNs, participates in care conferences, and helps ensure care plans reflect current physician orders.
