Job Description

VillageMD is currently seeking a Nurse Practitioner or Physician Assistant to join our home-based primary care team to support our vulnerable Medicare and Medicare advantage patients in Southeast Michigan (travel will be required to patients homes and can include but are not limited to: Livonia, Farmington Hills, Southfield, Westland, Dearborn Heights, and Canton, MI). Personal and professional skills for success:

Must be a graduate and certified from an accredited advanced practice program
Two or more years of primary care or home care clinical experience as an Advanced Practice Provider, with a demonstrated ability to work autonomously; capable of providing care without physician supervision
Comfortable treating adult patients, conducting comprehensive or episodic health history and physical assessment of patients/families with acute, chronic and/or potential health problems
Working knowledge of chronic care management and various medication options
Current, unrestricted State license or be eligible for licensure and valid drivers license
Active Drug Enforcement Agency (DEA) registration or be eligible for registration
Must possess active Medicare/Medicaid Enrollment or be eligible to enroll
Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
