Description

Job Summary:        

  • Provide care to patients according to plan under supervision of RN.    
  • Assess patients' condition making necessary adjustments in patients' plan of care related to symptom changes and changes in care needs.     
  • Act as liaison for patients/caregivers with MD and community healthcare partners.      
  • Complete all documentation related to patients' contact, assessment and care.     
  • Maintain understanding of current home care regulatory guidelines.    
  • Make necessary hospice visits as assigned.    
  • Demonstrate and encourage team behavior and exceptional patient/guest experiences.      
  • Uphold and promote patient safety and quality.     

Qualifications


Education:                
Graduate from an accredited school of practical nursing and currently licensed in the state of Alabama required.                

Experience:                
Minimal 2 years' experience in a hospital setting working in MedSurg, critical care or ER. Previous home care or hospice experience preferred.                

Additional Skills/Abilities:                
Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize schedule to meet patient needs.

Position Summary: Performs nursing procedures/treatments in the home within scope of practice. Works under the direction of the registered professional nurse. Essential Functions and Responsibilities: Performs a variety of basic nursing procedures under the direction of a registered nurse which may include, but are not limited to, wound care, wound irrigations, medication administration, catheterization, range of motion exercises and glucose monitoring. If the LPN is appropriately qualified, prepares and administers oral, rectal, subcutaneous, intramuscular, intradermal and topical medications to patients per policy and procedure. Observes patient response to medication, records unexpected reactions and reports them to the registered nurse. Identifies common patient problems, symptoms and reactions through monitoring Participates in the development, review and evaluation of the plan of care appropriate to the age of the patient in conjunction with the registered nurse. Demonstrates sensitivity to patient comfort and privacy and expresses interest in the patient progress. Provides for the physical and emotional comfort of the patient as well as a safe home environment. Required: Current licensure with the Michigan State Board of Nursing. Two years current nursing experience. Current CPR certification. Valid driver’s license, proof of valid auto insurance, and maintain a driving record in accordance with McLaren’s corporate driving policy and eligibility guidelines.
