Navigator RN - Population Health

Description

Under the direction of and in collaboration with the O&SM Regional Service Line Director, the RN Navigator Total Joint Replacement (TJR)/ Alternative Payment Model (APM) is responsible for navigating TJR patients through all phases of the care pathway (from the preoperative setting through a 90-day episode of care), and providing leadership on performance improvement (PI) teams to ensure success in APM participation and TJR Center of Excellence (CoE) designation. The Navigator optimizes patient, staff and provider satisfaction with the CoE program, while ensuring quality and cost benchmarks are achieved. A comprehensive understanding of TJR best practice standards, case management principles, APMs, effective education delivery methods, continuous process improvement and data management/reporting are key to performing this role.

Required Qualifications

  • Coursework/Training: Graduation from an accredited RN program.
  • Upon hire: Washington Registered Nurse License.
  • Upon hire: National Provider BLS - American Heart Association.
  • 2 years recent clinical experience in acute and ambulatory care orthopedics settings. Professional Orthopedic specialty certification (or equivalent) required within 2 years of hire date

Preferred Qualifications

  • Case management experience.
  • Proven working relationship with physicians, clinic personnel, and medical center staff, with demonstrated success in relationship building.
  • Proven leadership and change initiative experience, having managed resources either directly or within the implementation of a new initiative.
  • Experience in education program development and presentation.
  • Current knowledge of alternative payment models.

This is a combined posting for an Associate Clinic RN and Clinic RN. The requirements of each role are listed below under each associated title. The Associate Clinic RN provides comprehensive nursing care for patients in an ambulatory care environment based on individual physical, emotional, spiritual needs, and appropriate care strategies throughout the lifespan. This position facilitates care coordination by using the nursing process to assess, identify, plan, implement, and evaluate all aspects of care and teaching needs via direct and telephonic/telehealth or electronic encounters. This role works in collaboration with other members of the health care team. The Clinic RN maintains compliance with professional nursing standards and regulatory requirements and supports clinicians in delivering quality health care to patients. Actions will reflect the organization's core values. The Journey Clinic RN provides comprehensive nursing care for patients in an ambulatory care environment based on individual physical, emotional, spiritual needs, and appropriate care strategies throughout the lifespan.
